Free sports physicals

When: Tue., June 14, 4-7 p.m. and Wed., June 15, 4-7 p.m. 2022

Free sports physicals brought to you by Tri-State Memorial Hospital. No appointment is necessary. All attendees must wear a facial mask covering inside of the clinic. Please bring the required physical form from the school your child attends or will be attending. Be sure to have the demographics (name, birth date, address) completed before you arrive.
